The Goal
Health Navigators’
goal is to drive down the cost of employer-sponsored health plans by improving
the overall health of their employee populations and reduce the need to
continually shift additional costs to employees and their families.
The Method
Under the direction of
Health Navigators, employers develop strategies where their employees can
successfully identify diseases at early stages and participate in programs which
improve their health if they are determined to be “at-risk”. This process,
tailored around the unique needs of each employer, uses evidence-based medical
standards and proven interventions which effectively reduce both risk and costs.
The Result
Companies who formally
develop and implement early disease identification and
risk-reduction interventions succeed in improving the overall health of their
employees and reduce the cost of their health plan.
Health Navigators' Role
Under the direction of
Health Navigators, employers implement strategies where preventable diseases can
be identified at the earliest stages when interventions are least expensive and
most effective. Tailored around the unique needs of each employer, the
process uses evidence-based medical
standards and proven interventions. Health Navigators' diverse
experience and established credibility, coupled with strong relationships with
medicine, health benefits, insurance, health promotion, technology and advocacy
offer significant advantages to our clients.
Health Navigators will always look first to an employer’s local community resources (hospitals, physicians, dietitians, etc.) to help implement solutions before introducing outside resources to support their effort. However, if local resources are either unavailable or unwilling to partner with the employer, Health Navigators will negotiate with outside resources to accomplish the job.
